Primary Care Services

For adult patients (those who have stopped seeing a pediatrician, which typically happens between the ages of 18 and 21), we offer a full range of primary care services, including:

  • Checkups (including annual physicals, sports physicals and wellness visits)
  • Chronic disease management (such as that for heart disease, hypertension and diabetes)
  • Patient counseling
  • Patient education (on diseases, medications, medical procedures, healthy lifestyles, etc.)
  • Preventive care (including health screenings and vaccinations)
  • Treatment of common illnesses
  • Adult/senior primary care services (for patients ages 49 and older)

When needed, your primary care physician will refer you to a specialist for further care.

We offer adult/senior primary care services specifically for patients ages 49 and older.

Diabetes Care

The UM BWMG primary care team includes a diabetes nurse practitioner who helps patients with diabetes and prediabetes with medication management and adjustment and understanding their care plan. The nurse practitioner also provides education on topics such as blood sugar readings, home testing and annual lab work. They can also refer patients to endocrinologists as needed.

Patients must receive a referral from their doctor to schedule an appointment with the diabetes nurse practitioner. All appointments are in person.
