University of Maryland Baltimore Washington Medical Group, recognizing that your health care needs change as you get older, offers adult/senior primary care services for patients ages 49 and older.  In addition to our primary care providers, our team includes case managers, a social worker and a pharmacist.

We provide personalized care plans along with compassionate, dedicated support to help you age with confidence and be your healthiest self.

Services

Primary care services for our middle-aged and older patients include:

• Comprehensive exams (checking your gait [how you walk], listening to your heart and breathing rates, measuring your blood pressure, conducting a memory loss screening, and other checkups as needed)
• Geriatric assessments
• Help with completing an advance directive (which allows you to choose who can make healthcare decisions on your behalf in the event you are unable to do so yourself)
• Help with evaluating and accessing the best options for clinical trials, holistic therapies, home care and palliative care, among other specialized health services
• Access to specialty physicians when you need further expert care (cardiology, orthopedics, cancer, urology, etc.)

During appointments, we allow plenty of time to get to know you, review your medical history, answer your questions, provide education and explain next steps. We also explain preventive screenings and immunizations that are recommended for your age group or according to your health history.

Chronic Disease Management

As you get older, your risk of developing chronic diseases increases. Our providers diagnose, treat and manage a range of chronic conditions that affect aging adults, including:

• Dementia
• Diabetes
• Falls/balance problems
• Heart disease
• High cholesterol
• Hypertension
• Incontinence
• Sensory impairment

You will be referred to specialists for further care as needed.
