Primary Care Services

Your primary care doctor manages your overall health and wellness, including routine checkups, screenings, and preventive care. He or she is the person you call when you have a cold, notice a change in your health, or need an annual physical.

Think of your primary care doctor as someone who specializes in you. Your doctor gets to know you and your health history to help you maintain good health. He or she coordinates your health care with specialists (such as cardiologists, orthopedic surgeons or others) and maintains your prescriptions to keep you safe.

Transitional Care Clinic

At UM Capital, we have a specialty clinic for our Medicare and Medicaid patients who have recently been in the hospital. Our team will follow-up with you to make sure you are getting the care and support you need to avoid another hospital stay. For more information, call 240-677-3000.
