Primary, specialty or urgent care virtual visits
The information contained in the patient's medical record is owned by the patient. The medical record is the property of the hospital where the care was rendered, and that hospital is the custodian of the record. The patient does not automatically receive a copy of their medical record upon discharge from the hospital.
Medical records must be completed by all physicians involved in the patient's care prior to being copied and released to the patient or designee. This process could take up to 30 days. Upon completion of the process, the patient may receive a copy of their medical record and a fee may apply.
Patient-related information may be released to a physician or medical facility for the patient's continuity of care when requested. We encourage patients to request medical record information at least three to five days prior to any follow-up care.
All requests for the release of medical records to other parties must include an authorization form signed by the patient and/or legal representative and identify the hospital whose records are being requested. We will not accept a request for all medical records that are made out to "UMMS."

Request Your Medical Records Free of Charge via MyChart (formerly MyPortfolio)
You can now request your medical records for your personal use from any UMMS hospital using the MyChart patient portal. After entering a few key pieces of information, your records can be released to you through MyChart at no charge. Simply log in to MyChart and follow the links under the Health Tab under Medical Records.

You can also access your medical records without going online. Follow the steps below.

Please mail or fax your authorization release form. Note that all in-person requests are suspended during COVID.

Patients will not be billed for copies of medical records.

You should receive your records within 5 -7 days of receipt of your payment. If you do not receive your records within 10 days, please call 410-328-5706 and select Option 3 to reach a Release of Information (ROI) technician.
