Home Health

Chesapeake–Potomac Home Health Agency has been operating as a free-standing home health agency since 1995.

The agency offers skilled services to patients recently discharged from the hospital who reside in Calvert, Charles or St. Mary's Counties.

Referrals are made to the agency from hospital discharge planners, physician offices, insurance vendors and other community organizations. Chesapeake–Potomac Home Health is a full service agency providing nursing, physical therapy, occupational therapy, speech therapy, social work and home health aide services to patients in their homes.

To qualify for services, the patient needs to be in need of skilled, intermittent care and under the orders of a physician. Once an order for home health is received, service is usually initiated within 24 hours.

Chesapeake–Potomac Home Health is a non-profit agency operated by a partnership with Calvert Health, UM Charles Regional Medical Center, MedStar St. Mary's Hospital and the St. Mary's County Health Department.
