Community Health Needs Assessment and Implementation Plan
Pursuant to the Patient Protection and Affordable Care Act (ACA), hospital facilities must perform a community health needs assessment every three years and adopt an implementation strategy to meet the community health needs identified.
The Community Health Needs Assessment (CHNA) presents data and key factors impacting the health of Maryland's Eastern Shore Residents. These CHNAs and implementation plans were conducted jointly by the UM Shore Regional Health hospital facilities:
- UM Shore Medical Center at Chestertown
- UM Shore Regional Health at Cambridge (previously UM Shore Medical Center at Dorchester)
- UM Shore Medical Center at Easton

CHNA Implementation Plan
