Medication Management for Seniors
Managing the often many medications that seniors take can be difficult. And mistakes with medications can cause problems that send seniors to the hospital.
Our Population Health Team works to help people learn how to take and keep track of their medicines. The team’s pharmacist visits nine senior centers throughout Caroline, Dorchester, Kent, Queen Anne’s and Talbot counties once a month.
The pharmacist meets with seniors to help with many common medication issues. These include:
- Signing up for automatic refills
- Filling out paperwork for drug assistance programs
- Reviewing Medicare Part D coverage
