Education and Support for Patients with Diabetes

For in-person classes, social distancing is observed and masks are required in all hospital facilities.
Diabetes Self-Management Classes
Offered every month in Chestertown and Easton, this three-session program (totaling 10 hours) provides medical information and strategies enabling patients to manage their diabetes for optimal wellness.
A referral from a primary care provider is required, and registration is first-come, first-served. Class size is limited to five participants to accommodate social distancing requirements.

Individualized Instruction and Support Services
Our staff provides individual instruction and support for patients on the following topics:
- Carb counting
- Gestational diabetes – care during pregnancy and what to expect afterward
- Glucose meters, injectable medications, continuous glucose monitoring (CGM) and insulin pump management
- Medical nutrition therapy, providing an individualized comprehensive nutrition plan
A primary care provider's referral is required. Both in-person and telemedicine (phone call or video conference) are available for this support service.

Diabetes Support Groups
Due to the COVID-19 pandemic, diabetes support is now offered on a group basis via video conference on the 4th Tuesday of each month. These meetings are open to family members, caregivers and patients, and feature informative presentations by diabetes experts as well as fellowship and support for persons affected by diabetes.

Community Presentations
Our Diabetes & Endocrinology providers, certified diabetes nurses and diabetes educators are available to speak to groups throughout the five-county region on diabetes prevalence, treatment and management.

Our Diabetes Education Team
Diabetes Education Team members teach the various classes and work with patients individually to gain control of their diabetes. They also speak to community organizations and clubs and lead the Diabetes Support groups.
