Overview of Department Services
5 West has a multidisciplinary team approach in identifying needs and planning care for our patients. The team consists of the Patient Care Coordinator, R.N., Case Manager, Social Worker, Pastoral Care representative and O.T./P.T. Rounds are presented every morning with the R.N. reviewing each patient under his or her care, then receiving input from each area to develop an appropriate plan of care.
Staffing Model
5 West follows a staffing grid based on hours per patient day and average daily census. Ideally, our ratios are 4 patients to 1 R.N for day shift and 5:1 during nights. Assignments are given based on acuity.
