Leslie Clark, RN, MSN, LBSW

Leslie is the Assistant Vice President of Population Health at UM Upper Chesapeake Health. She has been a key player in developing, implementing and overseeing our outpatient Comprehensive CARE Center and is responsible for the care coordination for both inpatient and outpatient case management departments at Upper Chesapeake Medical Center Bel Air and Harford Memorial Hospital in Havre de Grace. Leslie has worked in the health care field for 25 years in various inpatient and outpatient settings as a licensed social worker, nurse and nursing administrator.
