Project Overview
The interdisciplinary team aims to develop, implement and evaluate an incentivized video-led exercise program targeting stable inpatient heart failure patients in a step-down progressive care unit. Project goals include increasing patient engagement in activity while hospitalized, decreasing deconditioning and assisting patients in learning safe exercises to improve mobility and activity tolerance as they live with CHF. Hospital metrics impacted could include discharge disposition and readmissions. Incentives will be sponsored through rewards that can be acquired through reaching exercise milestones.
