Badge:

The Badge Office is located on the first floor.

Hours of Operation:

• Monday - Friday 8:30am - 9:30am and 2:00pm - 3:00pm

Parking:

Midtown offers free parking to providers at the Midtown campus.

Please send Nicole the following information:

• Full name
• Complete address
• Make and model of car(s)
• License plate number
• State where your car(s) are registered
• Color of car

Please pick up your free parking card and hang tag from the parking office located inside the parking garage, attached to the Midtown campus, on the first floor.

COVID Vaccine and Booster:

Please send Nicole a copy of the COVID vaccination card.
